Health Assessment and Health Promotion Plan Essay
Abstract Constipation is a common condition that affects people of all ages. It may be described as a variation in an individualââ¬â¢s normal bowel habit with discomfort and diminished quality of life. Medical assessment is required as the underlying cause may be due to a serious medical condition. Managing patients with constipation presents many challenges to the health care professional, not only overcoming communication barriers associated with bowel habits but also because there is no universally accepted definition. Constipation is usually multifactorial, often with complicated underlying patho-physiology and it can be influenced by physical, psychological, physiological, emotional and environmental factors. Chronic constipation is one of the most common lower gastrointestinal disorders affecting people in America and is a key health concern for healthcare providers. This is mainly accurate for patients in high-risk groups such as the elderly, patients suffering from immobility, neurologically impaired patients and those with multiple health-care needs, as well as and those admitted to the hospital or residing in a healthcare facility. Unfortunately, constipation may be regarded as less important than other conditions commonly seen in general practice. Constipation has cost implications in terms of medications, containment equipment and nursing time. The following paper will investigate a patient suffering from constipation while developing a better understanding and approach of management for such ailment. Health Assessment and Promotion Plan Mrs. Burns a 64 years old female presents with chief complaint of being ââ¬Å"constipatedâ⬠. She states she has a bowel movement about every 3 to 4 days, feels the need to strain at defecation and her stools are hard and painful to excrete. She also has stated having frequent headaches, fatigue, a feeling of bloatedness and loss of appetite. As mention in the book, ââ¬Å"Physical Examination and Health Assessmentâ⬠by Jarvis, the aging adult frequently reports constipation signs and symptoms, such as reduced stool frequency (less than 3 bowel movements per week), and other common and troubling associated symptoms like straining, lumpy or hard stool, feeling of incomplete evacuation, feeling of anorectal blockage and use of manual maneuvers. Common causes of constipation include decrease in physical activity, inadequate intake of water, a low-fiber diet, side effects of medications (opioids, tricyclic antidepressants, and antacids), irritable bowel syndrome, bowel obstruct ion, hypothyroidism, and inadequate toilet facilities. Upon obtaining subjective data it was found that Mrs. Burns lives by herself after the death of her husband 6 months ago. She states no longer being able to eat as she used to when her husband was still alive and she cooked every day; now she usually eats by herself. She stated she has lost over 20 lbs. since her husbandââ¬â¢s dead. She also reported usually feeling lonely and very sad. Client also stated her usual meals of the day include breakfast starting with a cup of coffee and a slice of toast, lunch is usually eaten between 1 and 2 pm and include can soup or a tuna sandwich if she feels hungry and for dinner tea. Anorexia is a loss of appetite, and the purpose for obtaining information about signs and symptoms of anorexia is vital to prevent the dangerous psychological and behavioral effects on all aspects of an individualââ¬â¢s life. The individual can become seriously underweight, irritable and easily upset which can lead to depression and social withdrawal. Anorexia can also affect sleep and lead to fatigue during the day, as well as decrease attention and concentration (Prynn, 2011). Mrs. Burns does not often consume fruits and vegetables or other additional source fiber. She does not like the taste of water, so is very rare for her to consume it. She states not having trouble chewing, swallowing, or feeling nauseous or vomiting, but she likes to take naps after eating. Mrs. Burns also reported having signs and symptoms of abdominal pain located in the right and left lower quadrants of the stomach. The pain usually starts after the third day of constipation, which she describes as cramping (colic type) and usually relieved after she ambulates or has a bowel movement. Mrs. Burns describes her bowel habits as changed from going on a daily basis to only having a bowel movement every 3 to 4 days with a hard consistency. In her past abdominal history she reports not ever having an abdominal surgery. She brought an abdominal x-ray report, which concludes fecal matter to be present. She reports her list of medications including calcium, iron supplements and antacids, which she takes on a daily basis. The patient reports that being constipated all the time makes it really difficult for her to have a normal life. She reports her coping mechanisms as taking over-the-counter preparations especially laxatives, the use of digital stimulation and taking ibuprofen as necessary to relieve the pain when food or ambulation are not effective. A throughout functional assessment was performed and found that Mrs. Burns is able to ambulate, perform activities of daily living, including instrumental activities of daily living and has no problems with mobility. In the other hand, she has reported that she used to be much more active while her husband was alive and remembers walking the park for at least 20 minutes three times per week. Upon physical examination the following anthropometric measures and vital signs were obtained: Height: 162 cm (5â⬠²4â⬠²Ã¢â¬ ²), Weight: 65 kg (143 lbs.), Temperature: 36.2à °C (97.2à °F), Pulse: 82 BPM, Respirations: 20/minute, Blood pressure: 128/74 mm Hg, Active bowel sounds in all four quadrants and abdomen slightly distended without pain or tenderness at the present time. Gait and posture are normal for a patient of her age. There are no complaints related to lower back symptoms, perineal area observed free of any abnormalities or redness, perineal movement and anal sphincter squeeze noted with moderate muscle coordination. Digital rectal examination performed: hard fecal material noted, anal sphincter tone was normal, no rectal prolapse, no hemorrhoids, and no skin tags or anal lesions were noted. Labs results for hemoglobin, 11.8 and urinalysis, negative. Effective assessment provides nurses with the relevant information on which advice, interventions and management can be planned. In addition, it contributes to the path of outcomes measured and evaluation of care. Assessing patients with constipation presents many challenges to the health professional, not only by overcoming communication barriers associated with bowel habits and the embarrassment associated with an intimate rectal examination, but also because constipation may not result from a single straightforward cause. The subjective nature of constipation adds to the difficulty of the assessment, especially as nurses tend to use the objective measurement of bowel frequency rather than using a subjective symptom tool (Kyle, 2011). Assessment is based on a consideration of all the possible causes, while particularly assuring that it is not caused by an underlying undiagnosed medical condition. The aim of assessment is to establish a symptom profile in order to plan individualized bowel care. The main goal of treatment and management for constipation is prevention and relief. Establishing an ideal bowel action should prevent recurrence. Therefore, effective assessment provides nurses with the information on which advice and interventions of management can be planned effectively. Establishing a symptom proï ¬ le assist in identifying the most likely causes for the bowel symptoms based in the context of a more relevant medical/surgical/obstetric history and functional ability. Three main components have been identified as part of the plan of care developed for Mrs. Burnsââ¬â¢ current chief complaint. These primary components include: the implementation of an exercise routine, as well as a dietary regimen that will include more ï ¬âuids and ï ¬ ber. This is better known as lifestyle advice or step one of a stepped approach to bowel care, which is often recommended for promoting a healthy bowel and is still considered the ï ¬ rst-line treatment for constipation (Kyle, 2010). The implementation of this approached will be monitor by a dietitian, along with the nurses who will provide Mrs. Burns with dietary education and lifestyle modification strategies. In addition, other very important components will be included as well. Mrs. Burns will have a psychological consult as she is at risk for depression as evidenced by her husbandââ¬â¢s recent death and feelings of loneliness and sadness. Nurses should develop a more proactive and evidence-based approach to the prevention of constipation rather than continuing with the existing reactive response to this distressing symptom. Such an approach is dependent primarily on improving the education and the skill-base of nursing and those with whom they work. Finally, further research and discussions will add to the knowledge framework of such a significant condition, since so many complications are rooted from unhealthy digestive systems. Breaking All the Rules
In How To Succeed In Business By Breaking All The Rules it states that to succeed in business one must be able to use an unorthodox style which allows that person to break free from ancient ways of thinking and be able to use new ideas in developing entrepreneurial and business skills. One way this will be proven is to look at how positive thinking and motivation hamper people in making tough business decisions. Secondly, this will be proven by examining the education system, and showing that one does not always need a university education to perform in a business environment. Thirdly, this will be proven by looking at the behavioral aspects of an entrepreneur, and how an entrepreneur's thinking pattern is quite unique compared to the average person. Finally, this will be proven by looking at the illusions of management and how managers have to adapt if they want to see results. ââ¬Å"Forget just about everything you were told about positive thinking and motivation. â⬠Positive thinking and motivation can encumber persons in business if not executed properly. People assume that by being positive they are in turn motivating people to work hard and complete tasks without impeding their space. This is not entirely true. There is a joke that many motivational speakers tell, as an illustration of positive thinking: ââ¬Å"A guy has tripped and fallen off the roof of a thirty-story building. He is falling toward certain death. Some one yells out the fifteenth-story window, ââ¬Ëhow are you doing? ââ¬Ë And the falling fellow hollers back, ââ¬ËOkay so far! ââ¬Ëâ⬠This joke is not an accurate representation of positive thinking. It illustrates stupidity. This joke has taken positive thinking way out of proportion. It is okay to be positive when approaching tough situations, but a constant bombarding of motivational thoughts may in turn impede the situation overall. People are taught from the start that there are no bad ideas, and in-turn people who criticize these bad ideas are looked upon as ââ¬Å"negative thinkersâ⬠. Cynicism can actually be a healthy practice. By seeking out and eliminating bad ideas right from the get-go, the business or corporation is able appropriately deal with the problem and in a sense ââ¬Å"nip it in the budâ⬠. A person can think positive all that they want, but they are still going to spill coffee on their new tie. By over-motivating a person, the same effect can happen. A person has to want to be motivated for it to work properly. ââ¬Å"The Old Testamentâ⬠of motivation states that any person can do any thing as long as they are motivated to do so. This is not entirely true. In order for a person to be motivated they must believe the idea. Sounds like an oxy-moron, but a person needs to be inspired before motivation can take place. For instance, a person needs to want a post secondary education before they can be motivated to do so. A person does not necessarily need a university education to succeed in business. There is not really anything wrong with a university education as long as the person attending understands what it is and what it is not. For some careers a university education is essential. For instance, people do not look kindly to a self-taught brain surgeon. The problem is that a university education only offers you theoretical situations instead of practical life situations that can only be achieved through experiencing life. People tend to worry that without at least a degree in a particular field, they are unable to start their own lucrative business, and have the consumers take them seriously. Often people believe that without the proper credentials, it is impossible for their business to ever get off the ground. This is not true. There are many examples of companies that were started by supposedly ââ¬Å"uneducatedâ⬠people, who have turned their respective businesses into multi-million dollar corporations. For instance, Dave Thomas, founder and owner of the Wendy's Corporation. With nothing better than a grade 10 education Thomas took his first Wendy's that opened in 1969, and turned it into over 4000 locations across North America. Thomas was credited with hard work and life skills that helped him in his venture. From working as a busboy in a small town diner, to cooking for thousands as a chief in the army, Thomas learned the skills needed to create a lucrative business without any formal education. There are thousands of examples of entrepreneurs with no formal education that have made it in the business world. The most important thing to understand is that a business can only go as far as the people are willing to take them. Meaning that if you want to be successful you can encourage your self to take it to the next level without formal education. If you have a formal education, by all means make the most of it. But never use lack of formal education as an excuse and never let yourself feel inferior to those with better formal educations. There is abundant proof that you can reach just about any heights in business without college or even high school education if you will do the things necessary to otherwise obtain the information and master the skills specifically relevant to your objectives. Entrepreneurs are not always the best educated, but share a thinking pattern unlike those of regular people and are able to take any business with a solid foundation and turn it in to a productive money-making machine. In order to be a successful entrepreneur, one must be able to go against the grain of normal business practice and be able to perform at a pace where they leave the competition in the dust with no remorse. The old saying: ââ¬Å"there are no rules when it comes to love and warâ⬠also applies to entrepreneurs in business. An entrepreneur must be able show no compassion towards anybody in order to get the job done. It sounds cruel, but some of the most successful people in business today have no remorse for their competition. For example, Mark McCormack the creator of International Management Group (an athlete management corporation) started off with little more than a $1000 and a hand shake, but developed his business into a multi-million dollar corporation by leaving his competition way behind. McCormack was able to do something that had never been attempted before. Many companies fail to place a premium on the real dollar worth of their expertise, or what it would cost an outsider to learn what they already know. McCormack was able to market his knowledge to companies that were unaware of what they really had inside. By showing them what they already new, but by changing the way that they marketed the product McCormack was able to, almost over night, turn a small company distributing out of a barn, into a power house in today's sports entertainment industry. McCormack did not accomplish such a feat by caring about the little guy. In most cases, entrepreneurs are able to set aside their morals and values and go in for the kill. Successful entrepreneurs have a different way of approaching tough business decisions than the average person. They do not look at the situation or problem as how it affects the other guy, but how it affects them. When making tough business decisions, an entrepreneur must ask if solution best suits him, or if it best suits morality? In order for the entrepreneur to succeed, the solution must best suit his needs and not the needs of the other guy. They feel that by undercutting prices they are in-turn helping themselves to sleep better at night, but you cannot sleep if you cannot afford a place to sleep. In order to be self-sufficient, entrepreneurs must give up morales and values and be able to do what best suits them. The same is true for management in that if the want results from their workers then they must be able to adapt their style to best fit the interests at hand. There is a fairy tale about managers in business, especially big business: ââ¬Å"that corporate leaders are conservative, rational, deliberate people who arrive at important decisions only after carefully weighing a wealth of data and information. â⬠In most cases managers and corporate leaders rely on their staff to provide them with the information needed to make these tough business decisions. Mangers then are faced with the consequences if the result is less than ââ¬Å"kosher. â⬠As the old saying goes: ââ¬Å"if you want something done right you have to do it yourselfâ⬠applies when it comes to managing a top firm. In order for a manager to succeed, they must adapt their staff into a well-oiled machine that works for the most common goal. Managers have to set goals and limitations and make sure that their staff follow the guidelines appropriately for maximum production. Managers cannot baby their staff, as they cannot afford to lend the time in a high-speed industry or business. ââ¬Å"If you can't take the heat, get out of the kitchen. â⬠It is the managers responsibility to weed out those that are not able to perform with the rest of the unit, and in-turn hire people with the skills that best fit their own goals and ambitions. Managers also must be able to go on their own instincts and not always follow facts and figures alone. A good manager is able to assess the situation and then make a decision based on their gut reaction. The impact of Gorbatchev's reform Glasnost on arts and media between 1980 and 1990 - Research Paper Example In 1985 Mikhail Gorbachev became the Secretary of the CPSU and introduced the concept pr policy of Glasnost. Translated to English, Glasnost means openness. From 1985 to 1988 many interpretations were given by different sources for glasnost. While openness, public airing, and freedom of speech were the most commonly used interpretations, there were wordier interpretations. An astute interpretation put forward was that glasnost was the means by which Gorbachev intended to enlist the entire huge media of the country towards attacking the entrenched party workers and government bureaucrats that were so keen on profiting from the existing system of governance. ... as meant as ââ¬Å"maximum openness and truthfulness in the activity of the state and public organizations â⬠¦ But at the same time glasnost is not synonymous with universal permissiveness, the undermining of socialist values; it is invoked to strengthen socialism, the socialist code of morals(Gibbs, 1999, p.13)â⬠. Yet, in his memoirs Gorbachev himself that the glasnost that he had introduced with limited freedom aims, but ââ¬Å"glasnost broke out of the limits that we had initially tried to frame and became a process that was beyond anybodyââ¬â¢s controlâ⬠(Gibbs, 1999, p.14). 2.1. Influence of Glasnost on Visual Art Government censorship of art for several decades in the Soviet Union under the CPSU had led to a situation where genuine visual art had virtually become non-existent in the Soviet Union. The censorship introduce by the Soviet government was so harsh that it made sure that visual art could not become a means for registering agitation for political refor ms or as a platform for criticism of the government. Instead under pressure from the government visual art had become a visual tool and visual ally of the government. During the days of Stalin a non-conformist artist could end up in the Gulag or be put to death for non-conformist art was considered to be a part of political dissension. Professional development in this area is critical to a studentââ¬â¢s success in the classroom. Lastly, the proliferation of social media and technology should change the way educators teach, since the average student today is constantly using technology. How would you address a wide range of skills in your classroom? One of the classes that I used to teach was Spanish, there were a wide range of skills when it came to comprehension. Apparently, my co-worker wasnt pulling their weight in this area, I gently convinced them to attend a presentation that I was giving to a DCFS agency so that she can learn from my style of organizing and presenting the program to agencies. I found out during our outing, that she seemed to lack the organization in her presentation and just needed a few pointers and assurance in her recruitment efforts. The representation of family on TV The term nuclear family can be defined as a husband, wife and their two children; however in the last decade the framework through present television shows such as Modern Family Season 1-Episode 1, Simpsons episode Sweethome Homediddly Do and The Osbournes Episode Fight Club have begun to dismantle the hegemonic structure of the nuclear family when dysfunctional family members are torn from morally right decisions and left to their own deceptive behaviours. Modern Family, which was created by Sky 1, presents a very live action that contains gay parents and a dysfunctional family with a set of values. (Midgley, 2009). The Osbournes which, Ozzy is the rock and roll icon and father, does not take charge in helping the kids through conflict, but relents his role and gives his wife the responsibility of the children and household. Furthermore, The Simpsons, shows portrayal of hyper-vigilant mothers, who worry too much and the sorts of insane baby activities that occur at these groups was spot-on, and Homer and Marges junk food binge made up for some of the sloppiness in the main plot (Animation, 2009). The episode Sweethome Homediddly Do represents a comical view of a dysfunctional family which conflict arises then is resolved at the end. This essay will argue how Modern Family Season 1 Episode 1, Simpsons Episode Sweethome Homediddly Do and The Osbournes have manufactured dysfunctional chaos by making absurd decisions, reacting with wrong actions, coupled with out of control reactions, regretting their decisions with tremendous guilt and lastly, the constant shift in traditional power struggles are all challenging the boundaries of the hegemonic structure. These shows depict the underlining regression that has prevailed throughout societys deteriorating values to be acceptable and the norm of a nucleus family. ABC and MTV have addressed the overwhelming cry of broken relationships and to the rise of homosexuality, along with mixed signals of blended and single parenting society. The new direction of television sitcom is reflecting and desensitizing the family nucleus from what the truth is and what is morally right and acceptable. Families that are drawn to watch and to be entertained by dysfunctional chaos are now c hallenged to either accept or revolt to what they see. Modern Family is a show about when a family agrees to be interviewed by a documentary crew, they have no idea just how much theyre about to reveal about themselves. Another example which challenges the hegemonic of a nuclear family is the presence of the gay couple in Modern Family, (lawyer and breadwinner Mitchell and self-described stay at home dad/trophy wife Cameron) (Choudhary, 2010). This defeats the purpose of a well rounded nuclear family as seen as through the Clevers or Leave it to Beaver. Mitchell and Cameron have a adopted son who they both share and take care of, through the 21st century, television sitcoms are becoming more gay and lesbian icons which the viewers are adapting to. The hegemonic ideology of a perfect family seen as a father, mother and two children, show viewers that the family ideology has shifted dramatically due from Leave it to Beaver to a show like Modern Family which reinforce social attitudes of everything goes while breaking down moral prejudice that once was evident but now lost in society. Furthermore, Simpsons is portrayed through animation which contains a typical middle class family situated in the town of Springfield consisting of the character Homer Simpson, who is lazy, overweight, slow witted father with his hapless wife Marge; and their son Bart who is a underachieving and proud of it (Tueth). The Simpsons became a hit television show due to the framework of a dysfunctional but yet happy family. The episode Home Sweet Homediddly Dum Diddly starts off with Marge taking the leading role of a typical housewife which was represented in the 1950s, as a wife who can cook and clean. This type of class structure that is seen throughout this episode is a middle class American family that can only afford the necessities to live. For example in this episode, Homer gets Marge a pair of Spa gift certificates and she thinks he actually bought them while he got the Spa gift certificates for free for test driving a car (Groening, 2007). Another example from this episode is when Bart is getting his school picture taken then teacher Ms. Krabappel tells him to stop moving while she sees lice coming out of his hair (Groening, 2007). Homers Father is passed out lying on the coach with a mess everywhere; the child services crew comes and takes a picture and with that evidence the kids are taken to the child services. This is a key example how dysfunctional the Simpson family is and can relate now to majority sector of real-life modern family at a more subtle attack on the basic structure of the nuclear family of having a father that works and a stay at home mother who takes care of the children. Since Marge and Homer havent raised a normal family, their children also act in accordance with social norms, in essence complying with the accepted ideology of class (Jaffer, 2010). These are subliminal messages to the audience in the form of light-hearted entertainment via cartoons, not intended for children in their informative years of development. The last episode I will examine will be the Osbournes and how the Osbourne family challenges the hegemonic ideology of the nuclear family through their own life struggles as celebrities. Pieto states, the Osbournes, on the other hand, through their reality-based show, exemplify the American ideology of upward mobility. The reality of The Osbournes affluence is an ideological fiction for most working-class Americans (Pieto, 2009). Subsequently, Ozzy Osbourne known as Prince of Darkness is the front man icon from the 70s, as a pioneer of heavy metal music with a band called Black Sabbath, influencing audiences with drug use, sex and women. Ozzy brings home his abusive lifestyle and tries to stay on top of chaos with his slurred speech impediment and shaky hands, due to his rebellion of past drug and alcohol abuse. Ozzy as a figure tries to combine and tries to reconcile these two characters where you find on MTV and Leave it to Beaver. As seen through this episode of Fight Club of the Osbournes, the children Kelly and Jack do not get along and are always fighting; for example, they push each other non stop in the kitchen and always swearing at each other. The Osbournes have a similar narrative of a family sitcom where the conflict is resolved at the end. Furthermore, the realism that Osbournes represent to the audience is manufactured by the television producers, manipulating by editing and non-stop filming to get the desired results they want shown to the working class Americans and their pop culture of that era. As the ideological framework has been corrupted by 21st century, family sitcoms such as Modern Family, Simpsons, and The Osbournes, have challenged the idealistic family set of values introducing alternative lifestyles, blended families or single parent and furthermore same sex parents. These shows have become to dismantle and challenge the true meaning of a traditionally nuclear family. As seen through Leave it to Beaver is a prime example of a traditional nuclear family, the ideal American family a father and a mother, bound to each other by legal marriage, raising children bound to them by biology is a stubborn relic, a national symbol that has yet to be retired as threadbare and somewhat unrealistic (Benfer, 2001). Modern Familys gay couple Cameron and Mitchell are challenging the hegemonic ideology of the nuclear family every time it airs on television. They have taken one step further and have allowed scenes where the two men have kissed. These kinds of scenes are orchestrated to push the limits of what is allowed to desensitize the impressionable children growing up in this generation. Television is now targeting to more the Gay and Lesbian audience and is trying to show how a gay couple can actually live with an adopted child. This medium justifies what is already evident in our traditional culture and glorifies it to win the favour of its views. Furthermore, the animated sitcom Simpsons brings a humorous way of attacking the audience through the lazy Homer and the adolescence kid, Bart who consistently gets into trouble. Bart is looking for attention in the wrong way and as a child is behaving like an adult gone bad. His pranks and jokes are have an underlying connotation to them, demeaning his peers around him. Whereas The Osbournes shown through a celebrity and realism depicts Ozzys personality as a rock star and a father out of control. Sharon is the structure of the household keeping both kids and Ozzy in line, but their family is so dysfunctional there is always a conflict in the episode but the viewing audience never reaches a learning curve to their situations. It only leaves a distasteful appeal to critically analyze where did Osbournes success get them? As the 21 century seems to challenge the idealistic family set of values through living as single parents, unmarried partners or blended, gay and lesbian families as problematic (Benfer, 2001), this shows how the ideology has shifted dramatically and Modern Family, Simpsons and Osbournes have taken the ideology of the nuclear family and have set a different set of values to appeal to the viewers of the television. ABC and MTV has manipulated the television medium by giving a band-aid to its viewers, re-enforcing what families are dealing with, such as broken relationships and the rise of homosexuality in the nuclear family. The television producers manipulate by editing and selective script writing to get their message across to desensitize the family nucleus and to justify that this is the way our generation is. This enforces no family structure or accountability for unmoral lifestyles. Animation and realism intrigue the audience in the form of entertainment through the representati on of a comical viewpoint that entices the audience to want to view more. Constant exposure to dysfunctional chaos has the danger of excepting what the culture trend of our generation pushes, if we do not take action and be guided by our own convictions.
